NDIS Supports Built Around You
Before we draw up a single support, we sit down and listen. We want to know your routine, your interests and the things that matter to your day-to-day wellbeing. From there, our team works alongside you with consistency and care, whether that means help around the house, getting out into the community, or support through a major life transition.
What the NDIS Actually Does
Participants sit at the heart of the NDIS. You hold the choice and control over who supports you and how. Funding can be applied across a wide range of reasonable and necessary supports designed to build skills, encourage independence and open up everyday community life.
Breaking Down How NDIS Funding Works
Once your plan is approved, it outlines exactly what supports are funded and what they are working towards, whether that is more independence at home, stronger daily living skills, a pathway into employment, greater social involvement or improved wellbeing overall.
You choose how that funding is managed and who delivers your supports, which keeps everything aligned with your priorities and the life you want to lead.
Wondering if You Qualify?
You may be eligible for the NDIS if you:
- Are under 65 years of age when you apply
- Are an Australian citizen, permanent resident, or hold a Protected Special Category Visa
- Live with a permanent or significant disability that limits your everyday activities
- Need ongoing support because of your disability, both now and in the future
Joining Personalized Care, Step by Step
Shaping a Plan That Reflects You
The clearer you can be about your situation and what you are working towards, the more accurately your plan will reflect your actual needs. We are happy to help you prepare for that conversation.
Getting the Most From Your Plan
Understanding your funding categories and how best to use them means you can get genuine value from your plan and keep making steady progress towards your goals. Our team is always on hand to talk this through with you.
When Life Changes, So Can Your Plan
A major life event, a new goal, or the need for extra support are all valid reasons to talk about a plan review. Personalized Care will help you understand your options and prepare for that conversation with confidence.

How do you match me with a support worker?
We consider your goals, support needs, personality, interests, communication style and cultural background. Getting this match right matters to us.
What happens if my support needs change?
We review your supports on a regular basis and adjust your services as your goals, circumstances or funding arrangements change. Flexibility is built into how we work.
What community activities can you support me with?
We can support social outings, recreational programs, educational activities, community events, volunteering and skill-building opportunities tied to your interests and goals.
Do you support participants with mental health needs?
Yes. Where mental health supports form part of your NDIS plan, we can help with building wellbeing, confidence, social connection and everyday participation.
Can you help with transport to appointments, work or study?
Yes. We offer travel and transport assistance so you can attend medical appointments, get to employment or education, and take part in community and social activities.
What is the difference between Community Participation and Innovative Community Participation?
Community Participation is about taking part in and enjoying social and community life. Innovative Community Participation builds on this by focusing on new skills and opportunities that support your independence and long-term growth.
Do I need to self-manage my funding to work with you?
No. We work with participants who are self-managed, plan-managed or NDIA-managed, provided the relevant services are included in your NDIS plan.
Can my family or carer take part in planning my supports?
Absolutely. With your consent, family members, carers, guardians and others who play a key role in your life are welcome to join the planning conversations.
How quickly can my services start?
This depends on your specific needs and current availability. Once your initial consultation is finished and your service agreement is signed, we move as quickly as we can to get your supports started.
